Step 1
Referral Received
The process begins when a referral is made to the Complex Case Consultation Clinic. This can come from various sources, including other healthcare professionals, support coordinators, or even self-referrals.
Step 2
Service Agreement Development
Once the referral is received, a service agreement is developed, outlining the terms of the consultation, the expected outcomes, and any associated fees or costs.
Step 3
Service Agreement Review and Signature
The service agreement is then reviewed by both the client (or their representative) and the clinic. Once both parties are in agreement, the document is signed, formalizing the consultation process.
Step 4
Initial Contact and Booking
Within a week of accepting the offer for a Complex Consultation Clinic, the referrer or client will receive a phone call from the Clinic's administrative officer. During this call, a suitable booking time for the consultation will be organised.
Step 5
Include PBS plans, psychologist reports, assessments, etc.
Pre-Clinic Documentation
A Clinic background questionnaire is sent to the referrer or client. Along with this, they are reminded to provide all relevant reports, including Positive Behaviour Support (PBS) plans, psychologist reports, assessments, and any other pertinent documentation. This comprehensive documentation, which must be completed and returned at least one week before the clinic booking, ensures that the clinicians have a holistic view of the client's history and challenges.
Step 6
Clinic Preparation
Before the clinic appointment, the clinicians will review all provided documentation, ensuring they have a comprehensive understanding of the case.
Step 7
The Consultation Clinic
The 2-hour consultation clinic is conducted in person. The first hour focuses on case conceptualisation and understanding the issues, while the second hour is dedicated to strategising and formulating recommendations.
Step 8
Recommendations and Next Steps
Within a week of the consultation, a set of agreed-upon recommendations and/or next steps will be provided to all participants. This ensures clarity on the way forward and any necessary actions to be taken.
